Before I can treat your dog you must obtain consent from your vet for me to do so. This is a legal requirement for all physical therapies involving animals as specified by the Veterinary Surgeons Act 1966 and Exemptions Order 2015. It helps to identify any health concerns which may impact the treatment and is for your dog’s safety, as well as your peace of mind.

- return the form to me – either before or at your first appointment
- get in touch to arrange a date & time for your dog’s initial assessment
